We may collect contact details, practice specialty and structure, location, timing, broad priorities, buyer criteria, and business or aggregate operating information voluntarily submitted through our forms.
Our public forms and Workup intake are not designed for protected health information. Do not submit patient names, dates of birth, charts, notes, diagnoses, appointment-level information, or other identifiable clinical data.
Submitted information may be used to prepare a founder-reviewed Workup, respond to an inquiry, manage a controlled introduction process, maintain operational records, and improve the service. Analytics events record navigation and form-event categories, not form contents.
